Our Process

We have
A team of family physicians accepting new patients.

Please complete both steps to become a new patient with us:

Step 1: Registration Form:

Fill in a new patient registration form.
New Patient Intake Form
Step 2: Book your appointment with the doctor you prefer (see their info under "Doctors").

Book your appointment online and enter 'MEET & GREET' in the appointment reason and bring in a list of your meds. Please choose any of the doctors you wish to register with; Dr Kaliye and Dr Afzal are accepting patients to their family practice. You can see their details under the "Doctors" tab.

Book a separate appointment for any clinical issues you may wish to discuss.

All our doctors are registered with the College of Physicians and Surgeons of Ontario (CPSO) and the College of Family Physicians of Canada (CFPC).

Our family physicians are here to help with your ongoing healthcare needs. They can manage all your longitudinal care.

Walk-In Clinic services we provide

We offer same-day appointments you can book online or in person at our convenient Walk-in clinic location at 1709 Bloor Street West, located opposite the Keele TTC station.

Even if you do not have a regular family doctor, we can help to investigate and manage your acute and chronic health conditions.

Our goal is to provide care to those who are most in need of a family physician. If you our clinic is accessible for you, you are welcome to register.

 

To register, please complete two steps:

After completing the form, book your appointment online. Dr Kaliye, Dr Afzal and Dr Wakaas are accepting new patients please book with a doctor of your choosing

Be sure to enter "MEET & GREET" in the appointment reason and bring a list of your medications.

No, the "Meet & Greet" appointment is only for introducing yourself to the doctor. If you have any clinical issues you'd like to discuss, please book a separate appointment for those.
