Please complete both steps to become a new patient with us:

Step 1: Registration Form:

Fill in a new patient registration form.

Step 2: Book your appointment

Book your appointment online and enter 'MEET & GREET' in the appointment reason and bring in a list of your meds.

Book Appointment

Book a separate appointment for any clinical issues you may wish to discuss.

All our doctors are registered with the College of Physicians and Surgeons of Ontario (CPSO) and the College of Family Physicians of Canada (CFPC).

Our family physicians are here to help with your ongoing healthcare needs. They can manage all your longitudinal care.

Walk-In Clinic services we provide

We offer same-day appointments you can book online or in person at our convenient Walk-in clinic location at 1709 Bloor Street West, located opposite the Keele TTC station.

Even if you do not have a regular family doctor, we can help to investigate and manage your acute and chronic health conditions.

To register, please complete two steps:

Fill in the new patient registration form.

After completing the form, book your appointment online with either Dr. Ansari, Dr. Wakaas or Dr Afzal.  Be sure to enter "MEET & GREET" in the appointment reason and bring a list of your medications.

No, the "Meet & Greet" appointment is only for introducing yourself to the doctor. If you have any clinical issues you'd like to discuss, please book a separate appointment for those.
